Any medical relationship exists solely between the patient and an independent licensed provider.

3. Independent Provider

All medical evaluations, decisions, diagnoses, and prescriptions are made solely by an independent licensed provider.

SlimRx does not control, influence, or guarantee any medical decisions.

4. Independent Pharmacy

All prescriptions, if issued, are fulfilled by an independent third-party pharmacy.

SlimRx does not dispense medication or control pharmacy operations.

5. No Guarantee of Approval or Results

Submission of an intake form does not guarantee approval, prescription, or treatment.

All outcomes vary by individual.

6. Patient Responsibility

By submitting any information, you agree that:

All information provided is true, accurate, complete, and not misleading

You are solely responsible for all medical disclosures

You will not omit, falsify, or misrepresent any information

The provider relies entirely on patient-submitted information

SlimRx, the provider, and the pharmacy are not liable for any consequences, damages, or outcomes resulting from inaccurate, incomplete, or misleading information.

7. Limitation of Liability

To the fullest extent permitted by law, SlimRx shall not be liable for:

Medical decisions or outcomes

Provider actions or omissions

Pharmacy services or fulfillment

Patient errors or omissions

Any treatment-related outcomes

Each party operates independently and is responsible only for its own services.

8. Payments & Fees

SlimRx membership fees:

Are for administrative services only

Do not include medical services or medication

Are non-refundable unless otherwise stated

Provider and pharmacy fees are separate.

9. No Partnership or Joint Venture

Nothing in these Terms creates a partnership, joint venture, or agency relationship between SlimRx, the provider, or the pharmacy.

10. Eligibility

By using SlimRx, you confirm that:

You are at least 18 years old

You are legally able to enter into this agreement

You are providing truthful information

11. Modifications

SlimRx may update these Terms at any time.

Continued use constitutes acceptance of changes.

12. Governing Law

These Terms are governed by applicable laws in your operating jurisdiction.
